پروانه بهره برداری چاه آب

پروانه بهره برداری مجوزی است که کلیه واحدهای صنعتی که اقدامات لازم، شامل تامین زمین وتاسیسات و احداث ساختمان ، را برای نصب ماشین آلات خط تولید تامین نیروی انسانی مورد نیاز و تولید آزمایشی را انجام داده اند اعم از اینکه دارای جواز تاسیس بوده یا نبوده ، داده می شود.

پرسشنامه پروانه بهره برداري توسط متقاضي تكميل و پس از ارائه به سازمان صنايع و معادن استان ذيربط،مورد بررسي و ازنظرتكميل بودن اطلاعات مورد تائيد قرار مي گيرد.

رونوشت هاي آن به همراه تصوير پروانه بهره برداري به واحدهاي زير ارسال ميشود :

1دفتر آمار و اطلاع رساني وزارت صنايع ومعادن

2دفاتر صنايع تخصصي مربوطه

3اداره كل حفاظت محيط زيست استان – اداره كل كار و امور اجتماعي استان

مراحل مورد نیاز جهت صدور پروانه بهره برداری از منابع زیرزمینی :

1- ارائه درخواست و تشکیل پرونده:

اخذ درخواست کتبی از متقاضی و بررسی مقدماتی امکان تأمین آب(ارائه معرفی نامه از مدیریت جهاد کشاورزی شهرستان) ، ارائه مدارک مالکیت اراضی و تصاویر مدارک شناسایی برابراصل شده ،تکمیل فرم درخواست شماره 1 و تعهدنامه ها ،ارائه شراکتنامه و وکالتنامه ثبتی(درصورت داشتن شریک)

-2انجام استعلامات توسط امور آب منطقه:

استعلام از ادارات و سازمان های ذیربط توسط شرکت و پیگیری برای پاسخ توسط متقاضی

-3طرح پرونده در کمیته های مدیریت منابع آب:

ابلاغ مصوبه کمیته مدیریت منابع آب به متقاضی

-4کارشناسایی و بازدید میدانی از اراضی متقاضی:

اخذ هزینه کارشناسی ، تعیین تاریخ بازدید صحرایی با هماهنگی متقاضی ، انجام کارشناسی و تعیین محل چاه و تهیه گزارش و کروکی محل حفرچاه

-5طرح پرونده در کمیسیون رسیدگی به صدور پروانه ها (حفر) وصدورموافقت نامه انعقاد قراردادحفاری:

بررسی پرونده در کمیسیون رسیدگی به صدور پروانه حفر و ابلاغ نتیجه به متقاضی ، صدور موافقت نامه انعقاد قرارداد حفاری با شرکتهای حفاری مجاز ، ارائه قرارداد حفاری و صدور پروانه حفر

-6صدور پروانه حفر

صدور پروانه حفر و ارجاع پرونده به منطقه جهت تحویل پروانه حفر به متقاضی

-7نظارت بر عملیلت حفاری و پمپاژ

نظارت و انجام آزمایش پمپاژ ، تهیه شناسنامه چاه ، تعیین قدرت موتور پمپ با انجام محاسبات لازم

-8طرح پرونده در کمیسیون رسیدگی به صدور پروانه ها (بهره برداری(

بررسی پرونده در ستاد و طرح کمیسیون رسیدگی به صدور پروانه ها (بهره برداری(

-9صدور گواهی نصب منصوبات چاه و صدور پروانه بهره برداری

صدرو مجوز نصب منصوبات و کنترل و تائید منصوبات نصب شده روی چاه
